Mechanism of action Telmisartan is a member of a family of drugs called angiotensin receptor blockers (ARBs),. ARBs block the ability of the chemical angiotensin II to constrict or squeeze arteries and veins by blocking the Angiotensin 2 binding to the AT1 receptor. As a result, the arteries and veins enlarge and blood pressure falls. The reduced pressure in the arteries also makes it easier for the heart to pump blood. Telmisartan was approved by the FDA in November of 2000.
Amlodipine belongs to a class of medications called calcium channel blockers. These medications block the transport of calcium into the smooth muscle cells lining the coronary arteries and other arteries of the body. Since calcium is important in muscle contraction, blocking calcium transport relaxes artery muscles and dilates coronary arteries and other arteries of the body. By relaxing coronary arteries, amlodipine is useful in preventing chest pain angina resulting from coronary artery spasm. Relaxing the muscles lining the arteries of the rest of the body lowers the blood pressure, which reduces the burden on the heart as it pumps blood to the body. Reducing heart burden lessens the heart muscle's demand for oxygen, and further helps to prevent angina in patients with coronary artery disease.

Adverse Effects Telmisartan is well tolerated Headache, upper respiratory tract infection, dizziness, pain, back pain, fatigue, diarrhoea, sinusitis, influenza like symptoms, dyspepsia, myalgia, coughing, chest pain, urinary tract infection, nausea, pharyngitis, and abdominal pain have been reported.
ContraIndication Hypersensitivity to any ingredient; pregnancy and lactation; biliary obstructive disorders; severe hepatic impairment; severe renal impairment, cardiogenic shock unstable angina, aortic stenosis.
Special Precautions Renovascular hypertension; renal impairment and kidney transplant; intravascular volume depletion; other conditions with stimulation of the renin-angiotensin-aldosterone system; primary aldosteronism; aortic and mitral valve stenosis, obstructive hypertrophic cardiomyopathy; hyperkalaemia; hepatic impairment; active gastric or duodenal ulcer or
gastrointestinal pathologies, sorbitol; others.
Drug Interactions other antihypertensives, digoxin and lithium.
Dosage Once Daily.
