Product Description Glimepiride Glimepiride is the oral hypoglycemic drug of the class III generation sulphonyl urea. It is a very potent sulphonyl urea with long duration of action. Mechanism of Action Metgliforte distinctly lower the blood glucose level by both defects of NIDDM (ie ) insulin secretion and insulin resistance.
Glimepiride distinctly lower the blood glucose level by both defects of NIDDM, by stimulating pancreatic beta cells to produce more insulin and induced increased activity of peripheral insulin intra cellular receptor.
Metformin is an anti hyperglycemic agent which improves glucose tolerance in patients with type 2 diabetes, lowering both basal and postprandial blood glucose. Metformin decreases hepatic glucose production, decrease intestinal absorption of glucose and improves peripheral glucose uptake and utilization.
Glimepiride is effective as initial drug therapy. In patients where monotherapy with glimepiride or metformin has not produced adequate glycemic control, the combination of glimepiride and metformin may have a synergistic effect, since both agents act to improve glucose tolerance by different primary mechanisms of action.

Indications Metgliforte tablets are indicated to lower the blood glucose in patients with type 2 diabetes mellitus (non insulin dependent) whose hyperglycemia not controlled by diet exercise and Metformin or Glimepride alone.
Dosage Metgliforte 1mg or 2mg once daily.
ContraIndications Hypersensitivity to sulphonyl urea diabetic, ketoacidosis, renal disease or renal dysfunction, hepatic failure, Congestive heart failure requiring pharmacologic treatment, Known hypersensitivity to metformin hydrochloride.
Drug Interactions With NSAIDs like Salicylates, Sulphoamides, Chlorampenicol coumarin and probencid may potentiate the hypoglycemic action of glimepride. Thiazides, other diuretic, thyroid products, oral contraceptives, phenytoin tend to produce hyperglycemia. Drugs like thiazides and other diuretics, corticosteroids, phenothiazines, thyroid products, estrogens, oral contraceptives, phenytoin, nicotinic acid, sympathomimetics, calcium channel blocking drugs, and isoniazid may cause hypoglycemia. When such drugs are administered to a patient receiving metformin, the patient should be closely observed to maintain adequate glycemic control.
Special Precautions Hypoglycaemia, alcohol not to be taken. When initiating combination therapy, the risks of hypoglycemia, its symptoms and treatment, and conditions that predispose to its development should be explained to patients and responsible family members.
Adverse Effects Nausea, GI Disturbance, Vomiting, Anorexia, Lactic acidosis, leucopenia, haemolytic anemia, Headache.
